ZOLL emsCharts allows you to create Custom Assessments that can be documented on Page 8 under the Assessment Action category. You can add multiple questions to your Custom Assessments using a variety of Response Types ranging from Yes/No answers to multi-select lists and even free-text fields. In just a few steps you can create your own Custom Assessments to capture data important to your Services.
Note: You must have permissions to modify Command Facility Code Tables to create Custom Assessments.

Go to Configuration > Command Fac. > Code Tables > Assessment Setup. Refer to the Assessment Setup Code Table topic for instructions on modifying or adding new Custom Assessments and their questions.

If any of your Custom Assessments use a question Response Type of 'Checkbox/Multiselect' or 'Dropdown/Select List', go to Configuration > Command Fac. > Code Tables > Assessment Custom Field Options. Refer to the Assessment Custom Field Options topic for instructions on creating the picklist values for those Custom Assessment questions.
